Medical Cannabis Customer Information Cards

If you are a dispensary owner who received medical cannabis customer information cards from the Maryland Poison Center (MPC) and would like to request additional cards be mailed to your dispensary, please complete the form below.

Medical cannabis customer information cards are only available for dispensaries licensed in the state of Maryland. Families, educators, and others who want to learn more about cannabis can check out the "What You Should Know About Marijuana (Cannabis)" blog post on the MPC's eAntidote blog and download our edibles information sheet, which provides important information about edible cannabis products that contain THC or CBD.

PLEASE NOTE: Additional cards ordered through this form will be mailed to the address listed on file for your dispensary with the Maryland Medical Cannabis Commission. We cannot mail these materials to dispensaries located outside of our Maryland service area.
